Medicare Advantage members can pair up with ConcertoHealth

ConcertoHealth, a provider of healthcare services for Puget Sound-area patients with complex medical needs, is partnering with Medicare Advantage health plans to offer multi-location primary care, care management, and social services support to Medicare members.

Medicare beneficiaries began making their 2017 benefit selections Oct. 15 and have until Dec. 7 to finalize their decision. Beneficiaries can also choose ConcertoHealth as their primary care provider and may consider joining a Medicare Advantage plan, said Stacy Zierath, president of ConcertoHealth Washington.

“Choosing the right doctor is just as important as choosing the right plan,” Zierath said.

ConcertoHealth, which provides care at home, in a care center, a hospital or a skilled-nursing facility, makes it easy for Medicare beneficiaries to access the care they need when and where they need it, according to Zierath.

cAccess to healthcare services is easy with same-day appointments, on-site lab testing, prescription drug coordination and comprehensive exams, said ConcertoHealth officials. The company also offers care managers who help residents develop and maintain a personalized care plan with 24/7 access to care coordination, educational programs, and social workers for everyday needs.

In Washington, ConcertoHealth serves residents of King, Snohomish and Pierce Counties.
